A Change of Grade Form is used to report a grade change after final grades have been submitted. They are NOT necessary for a mid-term grade change, but are used to report either an error or grade change due to work submitted after final grades are sent. The form is accessed at http://www.aionline.edu/facultyform/change_of_grade_form.asp and should be filled out and submitted to the Academic Program Director in charge of the course where the change of grade will occur.
